THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
1. Our Legal Duty
Motion OS and Dr. Shim are required by federal law (HIPAA 45 CFR § 164.520) and Ohio State law (ORC Chapter 3798) to maintain the privacy of your Protected Health Information (PHI), provide you with this notice of our legal duties and privacy practices, and abide by the terms currently in effect.
2. How We Use and Disclose PHI
- For Treatment: We use your health history and physical assessment findings to formulate your 1-on-1 performance plan and coordinate care.
- For Payment: Upon request, we generate superbills containing diagnostic codes for out-of-network insurance submission.
- Healthcare Operations: Internal quality evaluations and operational management handled securely via Jane App.
3. Your Health Information Rights
Under Ohio law and HIPAA regulations, you have the right to inspect and receive copies of your clinical records, request amendments to inaccurate records, request confidential communications, and obtain an accounting of disclosures made by our practice.
